    Download, print and complete the registration papers below. When finished, please call the main office at 609-298-6900 x1210 to schedule an appointment to review them.

     

    Please complete every document listed below.

    You should have a total of 13 documents submitted:

     

    - ORIGINAL Birth Certificate with raised seal

    - Copy of Deed or Lease Agreement

    - First Proof of Residency (such as utility bill, driver's license, etc.)

    - Second Proof of Residency (such as utility bill, driver's license, etc.)

    - Transfer Card from previous school

    - Authorization to Release Student Records

    - Student Entrance Form

    - Bus Safety Contract

    - Universal Child Health Record

    - Immunization Records

    - Health History Questionnaire

    - Emergency Contact Form

    - Health Release Form
