•  Dear Parent/Guardians:

    Welcome to Chesterfield Elementary School!  We look forward to meeting our new kindergarten class of 2017-2018 at our annual kindergarten screenings later this spring  This meeting will serve several purposes.  It is not only an opportunity for you and your child to visit the school, but also gives us an opportunity to learn a little about your child to help their transition into Kindergarten.

     

    You will be mailed in April/May a version of this letter containing a time and date for your child’s screening.  At your scheduled screening time, please bring your child to the main office of Chesterfield School, along with the following documentation:

     

    • Original Birth Certificate of Student (w/raised seal)
    • Proof of Residence (e.g., utility, tax bills, lease, closing statement)
    • Health Forms:  Using the link below, please download, fill out and bring with you at your child’s screening appointment.  

    Please have these forms filled out by your child’s doctor.  If your child’s doctor appointment is after the kindergarten screening date, you may bring the form to our nurse after the doctor visit.  This form must be returned with an updated immunization record before your child can start school. 

    • Registration Forms: As with the health forms above, please visit the link below, download, fill out and bring with you at your child’s screening appointment.

    During your visit, our staff will assess your child in a variety of developmental areas in order to ensure they will have the best educational experience in kindergarten.  Four students will be scheduled each hour for this screening.  Each child will meet with our staff members at “stations” in the Media Center, while the parents will be completing paperwork.  The assessment will be conducted as follows:

    1. Our School Nurse will discuss your child’s health history and check immunizations.
    2. One of our Kindergarten teachers will assess readiness skills such as gross & fine motor (muscle use) skills, visual-motor integration, and some basic concepts.
    3. Our Speech Language Pathologist will assess each child’s communication skills.

     

    Please assure your child that these are friendly meetings designed to help them get ready for school.

    This will be the only opportunity for us to screen your child before the first day of school.  If you cannot make the scheduled date, please contact our main office at 298-6900. Thank you for your cooperation.
